6-501.111 Keep the premises free of insects, rodents, and other pests. Observed dead roach near back room handsink. Observed a large amount of mouse droppings in storage room and pistachio and sunflower seed shells that were being eaten by rodents. PIC stated pest control has been coming out at least monthly to set traps and treat facility. Floor under shelving needs to be cleaned of rodent droppings, shells, and other debris. |

Observed debris on floors in WIC, stock room, and room with 3-comp sink. Facility needs to clean the floors. |
