NOTE: No points deduction: #4. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).- Observed broken toilet seat, and broken handrail in girl’s restroom 1st grade hallway. Repair needed. # 6. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.- Observed water stain on ceiling tiles on second floor, in hallway or inside classrooms (215, 216, 222..). Check for no active leaks, repair if needed and replace the ceiling tiles. - Assistance Principle of operations: Idris kalkan present during inspection

Some hand sink in restrooms started to separate from the wall, new caulking maybe needed. Keep in good repair. Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).- Observed either soap and/or paper towel is not available in women staff restroom, second floor boy and girl restrooms. Ensure to provide all hand washing supplies at all hand sink at all time.
