Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed several toilets in boy's 7th/8th mod bathroom to be stopped up and/or with urine and feces in them. Ensure bathrooms are checked routinely for proper function and cleanliness. |

All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411. Observed 410 storage and 809 data room with items throughout space. Items are all directly on the floor. Observed bulk stock of toilet paper in boys bathroom 6th floor mod stored directly on floor. |

Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed chemicals including but not limited to windex, 409, and disinfectant wipes throughout facility within reach of children. Observed several essential oil diffusers. Ensure that oils being used do not have chemical warnings.
There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). Observed water table in courtyard of kindergarten/1st grade area with some standing water in it, drain and store so that it cannot collect water. Observed standing water between mod buildings. Ensure area drains. |
