Work order has been placed for missing holders. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). 6. Observed water stained ceiling tiles during inspection in classrooms. Needs to be replaced and ensure leaks are not ongoing. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Ms. Nancy Osvai (Executive Assistant) was present during inspection.
