These facilities shall be kept clean and in good repair.- Observed pink/gray/black mildews around shower basin need better cleaning and keep clean in Fraser room 2109, 2120, 2117. River Birch room 1106, 1121. Observed slow draining at hand sink room 1106. Observed stained of coffee in hand washing sinks in activity rooms, need better cleaning and keep clean. Observed cracked at basin of the hand sink in activity rooms throughout the facility, need repair or replace. |

.1312 (b) A sign shall be posted in each toilet room for institution employees, directing them to wash their hands after each visit to the toilet room and before returning to work. Observed no hand washing signs in one staff restroom. Hand washing sign shall be posted in all restrooms where staff have access to. No point escalation at this time. Due to improvement. |
