.1312 (a) All institutions shall provide toilet, handwashing, and bathing facilities that are available for use by residents and staff. These facilities shall be kept clean and in good repair.- Observed pink/gray/black mildews around shower basin need better cleaning and keep clean in room 1120, 1119, 2101. Observed stained of coffee in hand washing sinks throughout the facility, need better cleaning and keep clean. Observed cracked at basin of the hand sink in activity room 2nd floor River Birch, need repair or replace. No escalation in point due to noticeable improvement.
NOTE: The facility has tried really hard to keep all the shower basin clean. However the stained had become partially permanent. Until the replacement project happens, the cleaning crew is doing their best to keep shower basin clean as possible. |

.1312 (b) A sign shall be posted in each toilet room for institution employees, directing them to wash their hands after each visit to the toilet room and before returning to work. Observed no hand washing signs in the restrooms where staff have access at the back car porch restroom. Hand washing signs provided at the time of inspection. |
