No demerits were issued for the following: 5. Observed bag of pretzels in the after school room not properly sealed. Ensure that food items are placed in appropriate containers once opened. **Store all foods in approved re-sealable bags or containers once opened. 15A NCAC 18A .2806 (a) 25. Observed disinfectant at 250ppm at the diaper changing table. Ensure that disinfectant is between 500-800ppm to effectively disinfect surface. CRITICAL VIOLATION **The diaper changing surface shall be properly disinfected between each child. 15A NCAC 18A .2819 (c ). 32. Observed two mats within the cribs torn in the corners in the infant room. **Keep all mats, cots, and cribs clean and in good repair. 15A NCAC 18A .2821 (a). 49. Observed wood border splintered on the playground. Needs to be repaired. **Keep the premises clean and free of dilapidated structures and broken play equipment. 15A NCAC 18A .2832 (a).
