Observed temperatures above 41 F in prep RIC #3 (see temperature log). Prep RIC #3 was not cooling properly and had large amounts of water pooling at bottom of equipment. Food items determined to be eligible for salvage and immediately moved to cooler operating at 41 F. Prep RIC #3 needs to be repaired/replaced. V/R

Observed shredded cooked chicken that was date marked 10/23/2024 being stored in RIC. Shredded cooked chicken should have been discarded yesterday, 10/29/2024. PIC notified and instructed that RTE/TCS food items prepared and held more than 24 hours shall be discarded when held for a max of 7 days. Shredded cooked chicken was voluntarily discarded by PIC during inspection. CDI.

Observed flies throughout dry storage area and inside of establishment. Noted that fly curtain and fly light trap is not working. PIC notified and advised to repair/replace fly curtain/trap, or to utilize other method of fly control. V/R

Observed food items being stored directly on floor of WIF. PIC notified and instructed to remove food items off the floor. Advised that all food items shall be at least 6 inches above the floor.

PIC was notified and advised that wet wiping cloths shall be stored in between uses in a solution containing proper chemical sanitizer concentration. Solution was re-made with correct concentration of chemical sanitizer during inspection. CDI.

Observed wet-stacking of pots/pans on below prep table in dishwashing area. PIC notified and advised on proper drying methods. Pots/pans were re-washed, rinsed, sanitized and properly stored during inspection.

Observed significantly damaged gaskets on WIC door. Noted that WIC door itself was damaged. PIC notified and instructed to repair/replace WIC gaskets. Observed that Prep RIC #3 had large amounts of water pooling at bottom of equipment. PIC stated that it was condensation, but that water pooled enough to require someone to wipe it at least four times in a shift. PIC notified that unit was not cooling properly (see temperature log) and instructed to repair/replace prep RIC #3. Observed that RIF was damaged at bottom; bottom plate is missing. PIC notified and advised to repair/replace RIF bottom plate.
