General CommentsWhen checking records for food deliveries, observed one entry (Chief Salad) in the current month was received above proper cold holding temperatures. The log did not have a time listed of when the food left the preparation kitchen in Sylva. When asked about procedures for when food is received out of temperature, the PIC did not know what to do and did not know that 42F was above appropriate cold holding temperatures. According to log, the salads were below 41F when packed at the prep kitchen and were cooled back to 41F prior to being served.

The faucet did not have a backflow preventer installed. CDI- Had PIC remove the hose from the faucet. An air gap between the water supply inlet and the flood rim of the plumbing fixture shall be at least twice the diameter of the water supply inlet.

Need to have a trash can with a lid. A toilet room used by females shall be provided with a covered receptacle. 6-501.18 Cleaning of Plumbing Fixtures (C)- Observed the toilets in restroom have some slight build-up on the outside of fixtures. Bathroom fixtures should be clean to sight. recommend to clean. ~REPEAT~
