7-102.11 Common Name - Working Containers (Pf)- Observed a generic spray bottle of solution being stored with the chemicals without any labeling. When asked, the PIC was unsure what the substance was. CDI- Solution was voluntarily discarded. When removed from bulk containers, working containers of chemicals shall be labeled with the common name of the substance. |

5-202.13 Backflow Prevention, Air Gap (P)- Observed at the service sink, a hose was connected to the faucet and being stored in the flood rim of the sink basin creating a potential cross-connection. The faucet did not have a backflow preventer installed. CDI- Had PIC remove the hose from the faucet. An air gap between the water supply inlet and the flood rim of the plumbing fixture shall be at least twice the diameter of the water supply inlet. |

5-501.17 Toilet Room Receptacle, Covered (C)- Observed the women's restroom does not have a lidded receptacle. Need to have a trash can with a lid. A toilet room used by females shall be provided with a covered receptacle.
6-501.18 Cleaning of Plumbing Fixtures (C)- Observed the toilets in restroom have some slight build-up on the outside of fixtures. Bathroom fixtures should be clean to sight. recommend to clean. ~REPEAT~ |
