Observed hand washing sink in down stairs bathroom broken. Repair hand washing sink in downstairs bathroom. Observed no soap and paper towels at hand washing sink in bathroom and kitchen. Provide soap and paper towels at all hand washing sinks at all time.Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).Each resident will be provided soap and individual towels.
