[REPEAT] There were two packs of QAC sanitizer test strips available for use in back room at utensil wash sink. One pack was not yet expired (December 2026), but another pack had expired after 4/2025. CDI by employee voluntarily disposing of the expired strips. |

(REPEAT) Observed loose and damaged FRP wall paneling on wall base and damage to wall base as well between water heater and service sink and across from utensil wash sink in back room. Replace all cracked and damaged floortiles like those observed in front of the service sink in the back room.
