Inspection completed with Gay Gasper and Billy Robinson present. Inspection completed by Ariel Humphrey with assistance from Eli Freese. Intent to suspend permit issued for inadequate record keeping. Observed records being maintained only 3 days per week. Ensure records are maintained daily. Note- observed body hook wingnut on inside of shepherds hook. Recommend placing wingnut on exterior to prevent injury. Observed 1 missing ladder end cap. Replace.
