General Comments

22 - 911 operator indicated that EHS is not clearly audible. Operator turned up volume to hear. Was able to verify address and phone number. Recommend phone company looking at phone so 911 and EHS clearly audible. - Phone does not audibly ring back. Required repair within 30 days. Will check on return visit for ITS items. Equalizer lines not plugged per State recommendations. Per State recommendation, evacuate water from the line and then plug. Given 30 days to correct. Will verify on return visit for ITS items. Intent to Suspend issued for the following: .2535 - provide a test kit with new reagents .2528 - repair both gates to ensure no greater than 1/2" within 18" of push bar - between post and gate on either side .2518 - provide spare pump basket Site visit for in-field plan review.
