General Comments

Inspection completed with Jeremia Conradie. No signature due to COVID-19. Observed emergency phone operational but with no audible ring back. Advised CPO of this issue. Health Department recommends installing or repairing phone so there is an audible ringback. Observed bonding wire not affixed to pump. Bonding wire was reconnected during inspection. Intent to suspend issued for gate closest to pump room not self closing. Correct within 30 days by 9/23/2021.
