General CommentsPDSC form verified during inspection to be incorrect: - The current form indicates that the filtration pump is a Sta-Rite model # P4R6E3-187/ 1HP w/ an Emerson 1HP replacement motor. No identification found on pump itself; however, Sta-Rite cover on lint basket and replacement motor is an Emerson 1 HP. The pump appears to be a Sta-Rite Max-E-Pro model # P6R6E3-206/ 1HP w/ a flow rate of ~104gpm. Note: EHS observed that the 2 main drain covers are Aquastar WAV9WR and that the 2 equalizer line covers are Aquastar 6HP which match that listed on the current PDSC form. ***A new PDSC form must be completed reflecting the correct pump information and this new form must receive final approval by K. Wilson (MCHD Sr. Env. Health Spec.).*** IMMEDIATE SUSPENSION ISSUED for #2 & #3.
