Observed facilty has emergency dial box that is not operational and a working emergency phone. Box had previously been taped over as to not allow use, however tape was coming off and phone to be sitting on window ledge near box. Phone was secured to wall under emergency phone sign and box was re-taped over to allow for less confusion in the time of need. |
