Permit Restriction: Flow meter must remain operable. Flow shall not exceed 500 pm due to drain cover flow restriction. Suction Hazard. NIGHT SWIMMING IS NOT PERMITTED EHS observed that PDSC form states that motor is 3 HP. During in field verification, EHS observed that the pump is a 7.5HP motor. Facility shall update and resubmit new PDSC form within 2 weeks. If facility fails to submit the new PDSC form with updated information permit action may be taken. Main drain covers expire: 4/28/2029 Equalizer covers expire: 4/28/2029
