The following items need to be corrected: 1. Emergency phone: Need to be able to call the number and the box/phone connects, can hear the voice calling. 2. Pump needs to be grounded. Very Important. 3. Pump needs to be inspected and passed by an electrician. 4. Call main office to schedule an in field plan review 5. A new PDSC form will need to be filled out and approved. 7. Ensure Record book is on hand at facility. 8. Ensure there is a shower sign up and visible
