General Comments

#4 Observed food dated 5/27, 5/29 including egg salad sandwiches, pimento cheese, and tuna salad. Items were voluntarily discarded on site. **Once served , portions of leftover food shall not be served again unless the package is intact and the food is not potentially hazardous 15A NCAC 18A. 3308 (d)#34 Observed 2 sheets in the clinic exam rooms with slight debris build up. Sheet need to be kept clean and in good repair. #35 Observed outside table holding umbrella with crack in one of horizontal slates. Keep all furniture in good repair. Ms. Toni Maddox was present during inspection.
