Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed toilets in trailers stained and in need of cleaning. Observed toilet seat not attached in last stall in girls bathroom in trailers to the right of the building.
Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Observed wall edging in boys bathroom trailer coming off and in need of repair. Observed paint peeling on walls in girls trailer bathroom. Observed floor tiles separating in teachers bathrooms in A hall behind toilets. |

Observed floor edging coming loose and damage to walls in both trailer water fountain rooms. EHS observed mouse dropping in A hall. EHS observed pest control traps throughout building. Needs treatment to remove pests. |
