Observed multiple wads of paper, consistent with spitballs, adhering to the ceilings and walls in multiple restrooms throughout the school building. Observed writing all over the walls in several restrooms throughout. Keep walls and ceilings clean. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed toilets in need of cleaning in several areas throughout school and a broken toilet seat in girls bathroom near gym. Repair/keep clean.

Observed several hand sinks not operating in student bathrooms throughout school. Repair. Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c). Observed no soap in boys bathroom near main office and boys bathroom upstairs. Ensure soap is provided. Observed one hand dryer not operating in boys bathroom. Repair.
