NOTE: - No point deducted: #6. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.- Observed water damaged on ceiling tiles in hallway in front of cafeteria, 2nd floor in room B213, B216, B217, B218, B hallway, EC resource classroom in mobile unit MC350. Check for leaks, repair if needed and replace the ceiling tiles. Keep clean and in good repair. OUTDOOR PLAYGROUND: - Observed big/wide/few inches deep of old standing water between the two playground equipment outside. Attention Needed ASAP. Please get rid of standing water to prevent mosquitoes breeding at anytime. - Custodian: Ms. Emma present during walkthrough inspection. - Report review with head of custodian: Ms. Sedi and Principal: Mrs. Dozier.

4	20	TOILET FACILITIES: Walls and ceilings clean, nonabsorbent, washable, and in good repair 30#; fixtures clean and in good repair 40#; floors impervious, kept clean 30# (.2408)	No	No	No	REPEAT Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).- Observed disrepair toilet covered with black plastic bag in girl restroom 2nd floor. Observed several damaged/stained toilet seat in both boy and girl restrooms 2nd floor. Damaged toilet seats need to be replaced and keep clean. Keep all toilets fixtures in good repair.
5	20	LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409)	No	No	No	REPEAT Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).- Observed 3 loose/disrepair faucet at hand sink in boy restrooms, 1 loose/disrepair faucet in girl restroom 2nd floor need repair. Observed disrepair hand dryer in boy restroom by the elevator need repair. Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).- Observed no soap available in the dispensers in boy and girl restroom by the elevator, 2 out of 3 soap dispensers are without soap in boy and girl restrooms on the other side of 2nd floor. Ensure to provide soap/paper towel/working hand dryer for proper hand washing at anytime.
