A: Observed the restroom wall was damaged in C2 and RR795 boys. **Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).B: General Comment - Observed one toilet was clogged in B hall girls restroom. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).C: Observed the floor was repaire with plywood in RR101 boys, and the floors with litter in cafetera boys/girls restroom. **Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). |

A: Observed several sensor sink faucets were not working throughout the building: Cafeteria boys/girls restrooms, A hall boys restroom, RR101 boys; some push faucets did not operate properly. Adjust fixtures so water runs for at least 15 seconds. **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).B: General Comment - Observed one sink without soap (B hall boys restroom). **Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c). |
