Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).- Observed disrepair urinal cover with plastic bag in men’s restroom 5th grade hallway, disrepair toilet in one of the restrooms cover with plastic bag in 8th grade hallway. Repair needed. Keep all toilet fixtures in good repair.

Check for leaks/repair/replace ceiling tiles. Half point deducted due to improvement.
