NO POINTS FOR THE FOLLOWING VIOLATIONS: #4. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed a several toilets in school building "out of order", Per conversation with Head Custodian, a work order will or has been submitted to have those toilets repaired. #5. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Observed covering on spring loaded faucet missing on handsink in girls restroom located on third floor B hallway. *NOTE: Custodian, Mr. Ran, was present during time of inspection. Observed stained ceiling tile sporadically throughout school building. EHS advises school to ensure no active leak is occurring and to have stained ceiling tile replaced. Observed carpet staining in auditorium. Observed touch up paint needed sporadically throughout school building. Observed markings on walls sporadically in some restrooms throughout school building. Observed baseboard needing to be reattached in axillary gym.

Observed some shower tiles missing in boys shower stalls locked inside across from gym. Observed brown residue build up in shower wall of 1 boys shower located in basketball locker room. Observed detailed cleaning needed to showers throughout school building. Adequate facilities for storage of clothes and other personal items shall be provided and kept clean. 15A NCAC 18A .2413 (e). Observed shower stalls being used as places to store storage throughout shower stalls in school and outside football field house.
