General Comments

Angela Biddings (head custodian) was present during the inspection. NO POINTS WERE DEDUCTED FOR THE FOLLOWING: 9. Observed some showers and shower curtains needed cleaning in the field house locker rooms, and shower heads missing in shower stall in boys locker room. **All fixtures shall be kept clean and in good repair. 15A NCAC 18A .2413 (d). 11. Observed cleaning supplies and Lysol were stored on desks/white board in two classrooms (D206, and C301). **Store all chemical hazards out of reach of children as directed on product labels. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).

5	15	LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409)	No	No	No	Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Observed a sink was not working in women's field house restrooms, some push faucet sinks fixtures run for less than 15 seconds, and a sink was clogged in 3rd floor girls restroom (C hall). Adjust all push faucet fixtures so the water runs for at least 15 seconds. NOTE: Observed location of hand driers above the restroom trash cans; when driers are on trash is blown out of the trash cans. Recommend relocating the trash cans away from the hand driers.

Observed active leaks in the orchestra room, 3rd floor B hall, the carpet in the media center in need of cleaning, and the wall paint was chipping in the auxiliary gym.

Observed several light were not working in the girls locker room (3) and the dance room; accumulated bugs in the lights in the field house restrooms.

Observed one dumpster with no drain plug, all doors left open, and a mattress, broken mirror, and other debris accumulated on the ground surrounding the dumpster. **Keep dumpster doors closed and the dumpster area clean.
