Observed missing or peeling baseboards in 213, C14, and C10. Observed touch up paint needed in B9 and C18. Observed stained ceiling tile in art room. Fix leak and replace tile. Observed counter and back splash needing recaulked in C4 at sink. |

There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). Observed cockroaches(alive and dead) and cockroach droppings present in B9, C16, C18, C14, C2, and C4. Ensure pest control is done and that dead insects are removed. Observed spiders in therapy room along wall.
Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed aerosol air freshener, vaseline, and medicine stored on shelf or in open drawer that will not close in B9. Observed refill gallon of expo cleaner stored over clothes and briefs in 212 storage closet. |
