NOTE: #6. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. - Observed damage flooring in restroom mobile unit RR424. Observed some minor water stained on ceiling tiles, and several sagging ceiling in main hallway. Repair/replacement needed. - Head of custodian: Melvin Byrd present during inspection.

Several soiled looking hand sinks in restrooms hallways. Better detail cleaning needed and keep clean. Observed disrepair faucet at hand washing in restrooms mobile unit and main hallway girl restroom. Observed disrepair toilet in mobile unit. Need repair. Keep all fixtures in good repair. Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).- Observed empty soap dispensers and paper towel dispensers in restrooms mobile units. All hand washing supplies shall be provided at all hand sinks at all time.
