Observed walls and ceiling soiled in bathrooms throughout school building and modular unit. Thoroughly clean wall and ceiling in bathrooms. Repair floor in modular unit bathrooms. Cove moulding of flooring is damaged and is coming loose from wall. Observed protruding wire on base of girls bathroom stall door located down stairs. Observed stall door that will not properly open. Remove protruding wire from bathroom stall door and repair bathroom stall door so door will properly open. Tighten loose toilet seat in girls bathroom. Replace missing ceiling tiles in bathrooms and clean ceiling tiles! Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). |

Repair counter top in boys bathroom in modular unit. Tightly secure and seal back splash to wall and counter top behind hand washing in room E208.
