Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).- Observed broken soap dispenser in girl restroom in front cafeteria. Broken/or out of paper towel in the dispenser in girl restroom in front of cafeteria and girl and boys restroom 5th grade hallway. Please check all restrooms and hand sinks throughout and Ensure to maintain good equipment. Provide all hand washing supplies at all hand sink at all time. |

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed water damaged ceiling tiles in side stairwells 4th grade. According to maintenance, the leaks were fixed this past weekend. Replace the ceiling tiles, and keep in good repair. |
