*** Observed throughout the school water fountains need detail cleaning all around nozzles, spouts, crevices and surfaces. Keep clean. Observed in A hall there maybe leak from water fountains, floor is always wet need repair. Observed in C hall, low pressure of water fountains need to be adjusted. Observed in B hall disrepair water fountains need repair. Keep all water fountains clean and in good repair with adequate water pressure. |

*** Observed soiled/disrepair toilet/urinals in girl restroom B hall, boy restroom D hall, boy locker room and girl locker room. Observed stained toilet seat in girl restroom D hall, men restroom in the mobile unit, keep all toilet seats clean and in good repair. Repair all broken toilets and urinals, and keep clean.

*** Observed disrepair faucet in boy locker room, D hall boy restroom, cafeteria sink, and mobile units. Keep all faucet fixtures in good repair. Observed new caulking needed around hand sink in girl restroom C hall.
Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).
*** Observed no paper towels available at cafeteria sink, and wellness restroom at the front office, no soap available in restrooms of the mobile units, disrepair hand dryer in boy restroom D hall. Ensure to provide all hand washing supplies at all hand sinks at all times. |

*** Observed missing vent, damaged walls in the mobile unit restrooms need repair and replace. Observed soiled/dusted light shield covers through out need cleaning and keep clean. Some of light bulbs need to be replaced to provide adequate lighting in adaptive restroom in B hall, and band room. Observed flooring need cleaning in boy locker room, boy restroom A hall, A hallway. Keep all floors/walls/ceiling clean and in good repair. |

General Comment:
Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).
*** Observed throughout the school, classrooms have Lysol aerosol cans, all purpose cleaner spray container, Clorox wipes containers sitting out on tables/counters within reach of children. All toxic materials shall be stored out of reach of children as directed on the labels/instructions at anytime. |
