A) Observed the ceilings was water damaged in the 8th grade men staff restroom, a hole in the wall beneath a hand washing sink in the boys restroom near the gym, and the ceiling was leaking water in the girls mobile resstroom (R513).**Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).B) General Comment - Observed one cracked toilet seat at the staff restroom at the entrance.**Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). |

Observed school uniforms and P.E. equipement stored on the floor and the floors were dusty, the chorus rood storage area with dusty floor, the custodian closet floors with dust and debris, and some lockers with debris.**Storage spaces and custodians' closets shall be kept clean and arranged so as to facilitate cleaning. 15A NCAC 18A .2411. **All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.General Comment - Observed the custodian carts with paper stock stored beneath and with chemicals. **Re-arrange all carts so that the chemical and mop water are stored separate and beneath the clean paper supply (i.e. paper towels and toilet paper) to prevent contaminating clean paper supply. |

Observed lysol wipes, windex and otehr chemical cleaners stored within reach of students, buckets of floor stripper/finish stored behind a partition in the Auxillary gym/ESL, and the chemical prep room door was left open in room 614. **Store all chemical hazards out of reach of children as directed on each label.**Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). |
