-Repeat: Observed the ceiling in disrepair in the gym/fellowship hall.
-Observed a hole in the wall behind the door in the boy's modular restroom.
-Observed wood panel falling from the ceiling with nails protruding in the girl's modular restroom.
-Observed the carpet fraying and stained in the 9th grade room. *Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. |

-Observed a large bucket of floor cleaner and a container of Lysol aerosol spray stored within reach if the students.
-Observed aerosol spray bottle stored in an open classroom within reach of the students.
Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).
