A - Observed the B hall boys restroom walls was dirty at entrance, the B hall girls ceiling with tissue residue, the wall was damaged in RR539 girls (REPEAT NON-COMPLIANCE), and the wall near the urinals needs painting in RR539 boys. **Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).B - Observed a toilet base was dirty in the B hall boys restroom, and some toilet seats were loose in the restrooms. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).NOTE: R2 boys restroom is used for storage only. |

A - Observed one sink was not working in A Hall girls restroom, the sinks were not clean in R2 girls, the classroom sink was dirty in C9, the sink/counter laminate edge was missing in RR539 girls (REPEAT NON-COMPLIANCE), and some push faucet sink fixtures with very shirt run times (REPEAT NON-COMPLIANCE). **Adjust faucets so that the water runs for at least 15 seconds. **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).B - Observed no soap at the hand washing sink in B Hall girls restroom, and no paper towels in A Hall boys restroom. **Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c). |
