General Comments

Jerry Lisenby and Mark Thornton were present during the inspection.#11 - Observed one classroom with disinfectant wipes stored in a low cabinet. **Keep all chemical hazards stored out of reach of children as directed on product labels. **Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).#4B - Observed the mobile mens restroom(R2) is only used for storage, and the toilet seat was not secure in the A hall staff restroom. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).***Observed an un-vaccinated/un-restrained puppy (2weeks old) was stored under the desk in the nurses office. ***Do not allow un-vaccinated pets/animals on the premises.

5	15	LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409)	No	No	No	A) Observed the sink counter with missing laminate and wood rot in the boys mobile restroom (R539), 3 of 4 sinks in the girls mobile restroom (R2) were not working and the push faucet with very short run time, and the sink knobs were missing in the 1st floor modified restroom (repeat non-compliance). **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).
6	15	FLOORS: WALLS: AND CEILINGS: Floors, walls, and ceilings clean and in good repair 30# (.2410)	No	No	No	Observed the acoustic panels at the ceiling in the gym with accumulated dust, the soffit with wood rot/water damage and the exterior walls of R2 with chipping paint, and water damaged ceiling panels in room A2 and the media center.**Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.
7	10	STORAGE SPACES: Clean 20#; storage off floor 20# (.2411)	No	No	No	Observed boxes of supplies were stored on the floor in the 1st floor modified restroom, and boxes of toilet paper were stored on the floor in the gym storage closet.All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.General Comment - Observed one custodian cart with paper supplies stored below chemicals and on bottom shelf. Organize cart so that paper supplie (toilet paper and paper towels) are stored separate/above chemicals and mop water.
