Observed wall damaged around hand washing sinks in bathrooms(girls and boys). Repair walls around hand washing sink. Observed damaged caulking around hand washing sinks and hand washing sinks in bathrooms pulling from wall. Remove old damaged caulking and tightly secure hand washing sinks to wall and recaulk. Patch holes in wall and ceiling in upper 100 boys bathroom. Patch hole in wall in 600 boys bathroom and repaint. Repair damaged stall fixture in upper 300 boys bathroom so stall door will close. Wipe/clean stall fixtures throughout in boys and girls bathroom. Clean toilet paper from ceiling in bathrooms. Repair light fixtures in bathrooms or replace blown light bulbs. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). |

Observed water stained ceiling tiles throughout in hallway and classroom area. Repair any roof leaks, then replace water stained ceiling tiles. Repair roof leak at "horseshoe way". Observed water stains on wall from roof leaks. Observed sagging ceiling in hallway near entrance to gym. Check and repair ceiling tile grid holder in upper area of ceiling. Observed missing bar at door in hallway area near gym area. Replace missing bar to hold doors together. Clean upper section of wall in stairway area.
