Kelly Veloz was present during the inspection.General Comments:#9 - Observed some shoer stalls with litter and debris in the p.e. locker rooms. **All fixtures shall be kept clean and in good repair. 15A NCAC 18A .2413 (d).#11 - Observed the science prep rooms were left unlock, and one door left open. Prep room contained chemical solvent, flammables and other hazardous items. **Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).***Observed the water supply to the field restrooms was turned off. **Water must be available for cleaning.

4	20	TOILET FACILITIES: Walls and ceilings clean, nonabsorbent, washable, and in good repair 30#; fixtures clean and in good repair 40#; floors impervious, kept clean 30# (.2408)	No	No	No	Observed the urinal in the boys p.e. locker room was out of order(leaking), all of the toilet and urinal fixtures were dirty in the field restrooms. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).Observed the field restroom floors were with dead bug and debris. Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c).
5	15	LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409)	No	No	No	Observed all of the field restroom sinks were dirty, some restroom sinks with litter and debris, and one sink fixture was continuously running in the boys gym restroom. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).General Comment: Observed the exterior locker room hand washing sinks without soap and towels. The locker room is currently not in use/off-season. Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).
6	15	FLOORS: WALLS: AND CEILINGS: Floors, walls, and ceilings clean and in good repair 30# (.2410)	No	No	No	Observed the ceiling splatters with residue in the boys p.e. locker room, ceiling leaks. water damaged tiles in the library hallway and room 641, and some ceiling tiles were displaced in the 80o hall. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.
