Observed toilets soiled in boys and girls bathroom on B hall, in room A-2, A-6 and A-10 bathrooms. Thoroughly clean toilets in bathrooms. Observed blown light bulbs in light fixture in boys bathroom located on C hall and in girls modular unit bathroom. Replace blown light bulbs in light fixture. Observed wall and floor soiled in bathroom located on A hall. Observed ventilation soiled in bathrooms located within bathrooms on the A hall. REPAIR BROKEN WATER LINE SUPPLYING WATER TO MODULAR UNIT BATHROOM! Bathrooms were locked at time of inspection. THOROUGHLY CLEAN TOILETS IN MODULAR UNITS ONCE WATER IS REPAIRED! Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c).Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). |

Caulk/seal loose laminate on counter top in boys bathroom located in modular unit. REPAIR BROKEN WATER LINE SUPPLYING WATER TO MODULAR UNIT BATHROOMS! Clean walls throughout in classrooms around trash can area. Clean wall under computer desk in classrooms on B and C hall. Replace broken wall tiles in Whistle Stop storage room. Observed water stain on light shield in room C-12. Monitor and repair any roof leaks. Water stains should not be on light shield! Monitor water stained ceiling tile in C-9 and repair any roof leaks if needed. Observed ceiling tile near sprinkler head in room C-5 with microbial growth present. Repair leak on sprinkler system or repair roof leak. Observed crack along corner of wall in room C-4. Seal crack along corner of wall. Repair lower section of wall in modular unit MC188 and MC59. Repair floor in modular unit MC59. Floor is damaged near door in modular unit. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. |

Observed chemicals not properly stored in art room, A-2, A-10, A-12 and MC354. Observed raid bug spray in room MC354. Teacher informed me that modular unit contains roaches. Have exterminator contracted through school system to come out and address any bug issue! Recommend not having teacher to use bug spray in classroom area due to children with breathing/asthma issue. Store chemicals locked away or out of reach of children. Out of reach of children is not under cabinet area under hand washing sink within the classroom area. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). |
