Observed floors under toilets and walls behind toilets in girls mobile bathroom next to playground heavily water damaged. Observed wall peeling in boys bathroom next to playground and damage to partial wall holding sink. Observed walls peeling behind toilet in classroom 212. Observed cracks in floor in boys bathroom in mobile behind school. Observed dead flies covering floor in girls bathroom next to playground. Needs repair/cleaning. |

Observed chlorox and other disinfectant wiped, Lysol aerosol, febreeze aerosol, cleaners, and raid stored accessible to children throughout facility. Needs to be stored in accessible to children to avoid health hazards (as directed on manufacturer label). REPEAT. |
