Observed painting needed throughout facility. Facility is currently undergoing painting. Observed wall in auxillary gym where the concrete is degraded from what appears to be water damage. The paint is flaking off and soft throughout wall. Appears there may also be microbial growth on the wall as well. Observed floor tile in computer lab damaged. Observed flooring in 701 damaged. REPEAT VIOLATION |

Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed unlabeled spray bottle with chemical in it in custodial closet near gym. Observed room 402 had disinfectant wipes out. REPEAT VIOLATION |
