Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Observed the ceiling was damaged in RR644 men's restroom, wall damaged behind urinals in C hall boys restroom and wall base tile was missing behind the toilet in C hall girls restroom.
Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed toilets were soiled in RR1210 men's and women's restrooms, a toilet seat was loose in E hall girls restroom, and a toilet was not functioning in C hall boys restroom. |

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed one ceiling tile was not secure in E6, hallway ceiling tiles leading to the gym/cafeteria with water damage, a repaired wall was not finished with a coat of non-absorbant paint in B6 restroom, 3 missing ceiling tiles due to ceiling leak in A hall, and an unsecured ceiling tile in the nurse's office. |
