General cleaning of wall urinal in men bathroom in office building bathroom. Tighten toilet seat in 2's/3's bathroom within classroom. Repair toilet in 2'/3's bathroom. Toilet continue to flush. Repair urinal in upstairs boys bathroom. Patch holes in wall in bathrooms and repaint. Observed ceiling tiles throughout in bathroom made of absorbent material. Ceiling tiles shall be nonabsorbent. Repair flooring in upstairs boys bathroom. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). |

Observed table tops with damaged laminate. Replace damaged tables. Observed blue cushion chairs withs damaged covers. Replace damaged chairs. Observed damaged sleeping cots in 3's/4's room. Replace damaged sleeping cots. General Comment: Replace damaged board on climbing wall. All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).
