Observed broken soap dispenser in boy restroom 1st floor middle school, and boy restroom mobile RR 807, and girl restroom cafeteria hall. Ensure to keep all fixtures in good repair in order to provide hand washing supplies at all hand sinks at all time. |

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.- Observed water/mildew stain on ceiling tiles around AC unit in 200 hall, 300 hall, class 309, cafeteria, ecafeteria/main hallway, front office, class 604, middle school building 1st and 3rd floor. Observed missing/misplaces ceiling tiles in class closet 309, corner of cafeteria hall, 1st floor middle school building. Observed missing ceiling tiles, inside there is a vent cover with dust, need detail cleaning in middle school building 1st. Ensure to check for active leaks, repair, or/and replace the ceiling tiles. |
