Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed stained ceiling tiles in hallways, C112,C118, 100A, outside 102A, in 103A. Observed tiles missing in 5th grade boys bathroom at the bottom of the wall. observed boys mobile with wall damage and floor tile damage. Observed tiles broken on stage ramp. Observed torn wall to floor edging in A104. Observed wall paint bubbling/peeling in B111. Observed floor tiles cracked in C121 bathroom. Observed ceiling leaking in 100A. Needs repair. REPEAT |

Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed aerosols, disinfectant wipes and sprays, and other cleaners stored accessible to children in multiple classrooms throughout the facility. Needs to be stored to prevent health hazards.
