Observed walls soiled and stained throughout in bathrooms(gang bathrooms, bathrooms within classroom and modular unit bathrooms). Thoroughly clean walls in bathrooms and repaint walls in bathrooms. Observed microbial growth on ceiling in girls modular unit bathroom. Repair any roof leaks, then repair ceiling in modular unit. Observed base of toilets soiled/rusted from rusted bolts connecting toilet fixtures to the floor. Clean base of toilets. Clean stall fixtures throughout in bathrooms. Observed damaged stall fixture in boys bathroom on middle school hall. Base of stall fixture is rusted/damaged. Recommend replacing rusted/damaged stall fixture. Observed damaged stall door in girls bathroom on middle school hall. Repair stall door so door will properly close. Repair out of order toilet in boys modular unit bathroom. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Wipe/clean walls throughout in hallway area and classroom area. Observed water stained ceiling tiles in classrooms and hallway at far end of middle school hall. Repair any and all roof leaks, then replace water stained/damaged ceiling tiles. Observed roof leak and ceiling and wall damage in art room. Repair leaks and replace water stained ceiling tiles and repair and repaint wall in art rooms. Observed gaps along base of exterior doors. Recommend installing door sweep on base of exterior doors. Replace broken/damaged floor tiles on ramp and flooring behind gym area. Replace damaged moulding on steps behind gym area. Replace missing rubber cove base corner wall moulding in hallway area. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. |

Clean front of cabinet door at/near hand washing sinks within classrooms. Remove desk with broken base from ISS room. Replace damaged shelving under hand washing sink in room A 407 and any other rooms needed. All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b). |
