Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed toilets not operating in 3rd floor boys bathroom and girls bathroom across from 3002. Observed toilet not working in 2nd floor boys bathroom across from 2002. Observed urinal in boys bathroom in cafeteria broken. Observed toilet in JV field house locker room in need of cleaning. Work orders have been placed for broken toilets/urinal. REPEAT. |

Observed storage on the floor in storage and laundry rooms in field house. Observed gym storage with balls and other items on the floor. Observed boxes stored on the floor in a manner not to permit thorough cleaning in 1006 storage. Needs to be stored 15 inches off the floor or in a manner to permit through cleaning. |
