General Comments

Mr. Hayes (custodian) present during the inspeciton. No point deduction for the folloing: 5. Observed no paper towels were supplied in RR562 women's restroom. **Keep all hand washing sinks supplied with paper towels. 7. Observed the storsge closet piled with item and boxes/unorganized and items on the floor in room B3. **Keep all storage areas clean and organized with items stored above the floor. 8. Observed some lights not working in the men's RR562 restroom. **Keep light fixtures in good repair.

Observed the ceiling, walls, and doors with black residue in men's RR562. 5	15	LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409)	No	No	No	Observed a sink faucet was damaged (not secured to sink) in the gym boy restroom, a sink with pink residue in room E3, some push faucet with short run times (less than 15 seconds), and a sink was leaking in RR562 men's restroom. **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).

Store all hazardous items out of reach of children as directed on product labels.
