Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed several toilet out of order to include the following areas: Gym building second floor 1 toilet in girls restroom. Fine Arts Center 1 toilet in girls restroom. 1 toilet in girls restroom on B wing near mini auditorium.

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed several stained ceiling tiles to include the following areas: Fine Arts Center girls and boys restrooms, gym locker rooms both boys and girls. Observed hole in wall in boys restroom located on second floor of gym building. |
