General Comments

NO POINTS FOR THE FOLLOWING VIOLATIONS: #9. Floors, walls, and ceilings of dressing rooms and showers shall be kept clean and in good repair. 15A NCAC 18A .2413 (a). Observed black build up to walls in boys locker room shower, per conversation with Mr. Libby it is not mold rather residue from having floors redone. EHS recommends facility clean off walling and repaint if necessary. #11. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed aerosol Lysol stored in classroom C210 in reach of children. Observed over 6 bottles of hand sanitizer teacher classroom located in elementary wing. EHS advises that school building only have 1-2 bottles of hand sanitizer out per classroom. *NOTE: Director of Maintenance, Stephen Libby, was present during time of inspection. 6	30	FLOORS: WALLS: AND CEILINGS: Floors, walls, and ceilings clean and in good repair 30# (.2410)	No	No	No	Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed stained ceiling tile sporadically throughout school building to include but limited to: walkway outside of auditorium, upper C wing hallway, boys locker room in gymnasium, boys restroom located in theater building. Observed stained carpet throughout school building to include rooms C109 and B209. Per conversation with Mr. Libby, all flooring was deep cleaned over the summer and stains are old stains that will not come out of carpet.
