Observed dysfunctional toilets/uninal, the toilet stalls kept closed with tap or lock from the inside, urinal covered with plastic bag (boy restroom S hall, girl/boy restroom H hall, J hall: girl restroom 2nd floor and 1st floor- girl/boy restroom, C hall: girl/boy restroom, mobile unit: boy restroom. Repair needed and keep all toilet fixtures in good repair. Observed stained/ring circle in toilet bowls in J hall restrooms, detail cleaning needed and keep clean. |

Observed dysfunctional hand dryer (J hall boy restroom, E hall girl restroom, A/B hall girl restroom, C hall girl/boy restroom. Repair/replace needed.
Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).- Observed no paper available at hand sinks in boy and girl restroom. Provide all hand washing supplies at all hand sinks at all time. |

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.- Observed old water stained/ black microbial on ceiling tiles in throughout J hall, L hall, K hall, cafeteria, missing ceiling tiles in classroom K5. Check for no active leaks, repair if needed, replace the ceiling tiles. Keep all ceiling in good clean condition. |
